Smartwatch-based workplace wellness programs offer significant potential to improve employee health and reduce organizational healthcare costs, they also create substantial privacy risks through continuous biometric data collection. Understanding how to balance these health promotion benefits with privacy protection is critical for developing ethical and effective wellness initiatives. However, research on the privacy trade-offs employees face when participating in workplace biometric monitoring programs remains limited. Current frameworks inadequately address the tension between voluntary participation and economic coercion, the complexities of informed consent, and the effectiveness of different privacy protection models in workplace contexts. The main objective of this qualitative study was to explore the privacy implications of smartwatch-based workplace wellness programs by examining employee acceptance factors, evaluating alternative privacy protection frameworks, and analyzing regulatory and ethical considerations across different jurisdictions. Data were generated from comprehensive literature review and framework analysis, examining empirical studies on employee perceptions, technology acceptance models, regulatory comparisons, and ethical assessments. The analysis was conducted through the lens of the Privacy-Enhanced Technology Acceptance Model and self determination theory. Four distinct privacy frameworks were systematically evaluated: Minimal Collection, Consent and Control, Privacy by Design, and Federated Data models. The analysis revealed several critical insights. First, employees recognized the health benefits of biometric monitoring (62% viewed it positively) but simultaneously expressed strong privacy concerns (78% worried about data misuse). Second, the concept of "manufactured consent" emerged, where financial incentives and social pressure undermined truly voluntary participation. Third, no single privacy framework provided optimal protection; hybrid approaches combining multiple models were most effective. Fourth, trust in employers and vendors significantly moderated the relationship between privacy concerns and program acceptance. The findings suggest that organizations should implement hybrid privacy protection strategies, establish robust governance structures with clear accountability, provide genuine non-monitoring alternatives for earning incentives, and strictly limit biometric data use to wellness purposes. Furthermore, policymakers should develop comprehensive biometric privacy legislation that recognizes workplace power dynamics and mandates equivalent non-monitoring pathways. Employees should remain informed about their rights under applicable privacy laws and carefully evaluate the voluntariness of participation given financial and social pressures.
